The Community Rehabilitation Service (CRS), formerly known as the Integrated Rehab Teams (IRT), is a county-wide service. It is available to all adults who are having problems carrying out everyday activities due to a medical condition.

Service users will need to be motivated and willing to engage in therapy. CRS will aim to help people increase their level of independence by helping them regain or adapt their functional abilities to manage the impact of their condition.

They will be supported to work towards improving their ability to do the things that matter most to them. We do this through personalised rehabilitation plans and collaborative goal-setting. This can be carried out within the service user’s usual place of residence or other appropriate community settings.

The team consists of Physiotherapists, Occupational Therapists, Pharmacy Technicians as well as Rehabilitation Assistants, Assistant Practitioners and Advanced Clinical Practitioner (ACP) development roles.

The team also supports apprentices as well as undergraduate students throughout their training and work closely with other professions and services within the health and social care system.

Referrals can be made by one of the four admin hubs by email or phone (see details of areas below) and will be triaged for suitability and to establish how quickly the person needs to be seen.
