Living well after stroke

The living well after stroke team are based in the stroke rehabilitation units and are part of the integrated community stroke service.

We understand that stroke can affect the whole household, extended family, and friends as well as the patient themselves. We hope to be a point of contact for all who may need us. We can advise on how to access community services as part of living well after stroke.

The Integrated Community Stroke Service (ICSS)

The Integrated Community Stroke Service (ICSS) is a countywide, stroke specialist rehabilitation service operating from three hubs (Williton, South Petherton and Shepton Mallet).

Who is on the team?

The living well after stroke team are specialist health care professionals and support workers in stroke care. Our role is to be an ongoing point of contact and emotional support for you. Also, we will help you to understand the risk factors for stroke and support you to make healthy changes.

What can I expect?

The living well after stroke team can:

  • be there to answer any stroke related questions
  • ensure medical investigations are completed
  • offer emotional support
  • support family and carers
  • provide support for healthy living
  • assist with access to support agencies
  • be a source of information and education about stroke and the recovery process.

How can you access the living well after stroke team?

You can contact the team at any point after your stroke. This might be with questions or if you are anxious, worried, or need other support. Contact details can be found on the webpage.

  • You can contact the team as soon as you need to after coming out of hospital.
  • Everyone is offered a 6 month stroke review with the living well after stroke team. This is to discuss your ongoing progress. This may be over the phone, at home or in a clinic.
  • You may want further appointments to help you and your family adjust to the changes associated with a stroke.
  • The living well after stroke team will discuss with you any further health or social needs, to support you to live your life after a stroke.

What can families/carers do to help?

It is wise to seek support for your own physical and emotional needs in the first instance, so that you are fit and well to help support your loved one with their stroke recovery.

Helping your loved one with their stroke recovery may involve supporting them to complete rehabilitation exercises inbetween therapy visits.

The next stage

As your confidence builds and you can see a way forward in your life after stroke, a member of the team will encourage and support you to access local, community-based clubs and groups to continue your recovery.
