Our diabetes intermediate care service is a link between your GP surgery and Somerset’s district hospitals. It provides support and education if you are living with type 1 or type 2 diabetes.

Our team is made up of a nurse consultant, diabetes specialist nurses and diabetes specialist dietitians, all supported by our admin team and service manager.

We offer specialist advice, support and education sessions to help you manage your diabetes. We also work to raise awareness of diabetes in our community.

We run group DESMOND sessions (diabetes education and self-management for ongoing and newly diagnosed) across the county for patients who are newly diagnosed with type 2 diabetes.

Once you are referred to our service, we can carry out a detailed assessment and offer treatment and advice based on your individual needs. We also work with you to develop a tailored care plan and give you information such as leaflets, websites and reading to help you manage your condition.

How to refer 

You can access this service via your GP surgery through their e-referral service. But if you are housebound and have trouble coming to clinic then your GP or practice nurse can refer you direct to the service and the team will organise a visit to your home.
