Rapid Intervention Team

Part of Somerset’s Community Team for Adults with Learning Disabilities 

The Rapid Intervention Team (RIT) provides flexible support for adults with learning disabilities who are at rising risk of crisis or unplanned hospital admission. RIT works with individuals, families, carers, and partner agencies to strengthen support plans, respond to emerging concerns, and help maintain safe community‑based care.

What we do

RIT supports Somerset’s dynamic support and admission prevention pathway by offering:

  • County‑wide responsive support when risks are increasing and additional coordination is required.
  • Clinical oversight of the Dynamic Support Register and Blue Light Meetings.
  • Joint working with health and social care colleagues, including the REACT Team.
  • Liaison with emergency services and forensic health teams, where relevant.
  • Health care coordination/keyworking for people with complex needs, including those detained under the Mental Health Act or placed out of area.

Who we support

RIT works with adults aged 18+ who:

  • Have a diagnosed learning disability, and
  • Are experiencing increasing concerns that may raise the risk of crisis or hospital admission.

This may include situations where support arrangements are becoming less stable, but hospital‑admission risk is the primary trigger for RIT involvement.

Support is tailored to need and may be short‑term, intensive, intermittent, or long‑term.
