Emergency Referrals

Emergency referrals should be discussed with the on call Maxillofacial Surgery SHO who should be contacted via Musgrove Park Hospital Switchboard on 01823 333 444.

 

Suspected Head and Neck Cancer Referrals

Referrals from Dental Practices

All referrals from dentists should be made using the appropriate referral form. Referral forms and guidance are available at:

There are four referral forms and a supplemental GA request:

Referral for General Oral Surgery

Referral for Impacted Third Molars

Referral for Apical Surgery

Referral for Soft Tissue Lesions

 

If the referral is requesting general anaesthetic the supplemental GA Request form will need to be completed and submitted with referral form.

Please note we do not provide sedation in Secondary Care.

Referrals from GP Practices

Send a referral letter via NHS e-referral system.

Routine referrals for dental extractions will not be accepted from GP Practices – patients will need to see a dentist for assessment and radiographs to be referred.
