In-patient referrals for Yeovil Hospital should be submitted via the track care system.
Care Homes are now able to directly refer residents to our service.
You will need to provide evidence from the Dysphagia Management Checklist at the time of referral along with any swallowing diaries that have been completed.
Referrals made without referencing the checklist will not be accepted. You will then need to re-refer your resident with the required information.
