How you can access your records

You have the right to obtain a copy of your personal data from Somerset NHS Foundation Trust by submitting a “subject access request”. If you would like to exercise this right, we recommend that you complete our Personal Data Request Form, as this will help us to respond to your request fully and efficiently.

You may also request records relating to a deceased person, by completing our Information About the Deceased Request Form.

We aim to acknowledge all requests within two working days of receipt and provide a response within a calendar month, as required by law.
