Please note – if you are making a request on behalf of another person aged 12 or over and you do not hold Power of Attorney for that individual, we will write to the data subject to advise them that you have requested their records and to confirm their consent to release their records to you.

• Council tax bill/demand letter/exemption certificate, showing data subject’s name and current address (issued within last 12 months)
• Tenancy agreement from local council or housing association, showing data subject’s name and current address (dated within 12 months)
• HMRC tax notification showing data subject’s name and current address (dated within 12 months)
• Bank statement showing data subject’s name and current address (dated within 12 months)

List C

• Certified copy of Power of Attorney

List D

• Child’s Birth Certificate (with your name listed)
• Parental Responsibility Order from the Court
