How to refer a child or young person

Any professional or parent can refer a child to the Children and Young People’s Therapy Service.

GPs – Please note that if you are referring a child over 8 years old for musculoskeletal problems, it would be more appropriate for them to be seen at their Physiotherapy Outpatient Department.

Please note that this form times out after 20 minutes, so have all the information regarding the referral to hand.
