Ways to give your feedback

• Speak to the person providing your care
• Contact the Patient Advice and Liaison Service (PALS)
• Share your feedback via Care Opinion or by calling 0800 122 31 35
• You can also discuss the following feedback options with the person providing your care:
  • complete the NHS Friends and Family test
  • complete a Patient Experience Questionnaire (PEQ)
  • join our patient (also known as ‘service user’) participation service.
