Referring to Community Dietetic Services

Information for GP’s and Healthcare Professionals

Adult Community Dietetic Services

GP’s and health care professionals can refer directly to the adult community dietetic service using the downloadable referral forms below.

Patients can also be signposted to our self-management website www.patientwebinars.co.uk where appropriate. We currently have self management resources (webinars, videos and downloadable resources) for the following conditions:

  • Malnutrition
  • Irritable bowel syndrome (IBS) – including the Low FODMAP diet
  • Inflammatory bowel disease – for patients in remission
  • Coeliac disease
  • Constipation
  • Diabetes
  • Food Allergy
  • Kidney health
  • Weight management (coming very soon)

We also enable patients to self-refer using the form on our website for the following conditions:

  • IBS
  • Coeliac disease

For malnutrition/nutrition support referrals please read our MUST care pathway for community prior to referring. Note: we now have a separate pathway for care homes (see below)

Electronic Referral Forms

Please note if you choose to use our electronic referral forms you will need to download and save them first before typing into the form fields (otherwise the text does not save). If preferred please use the standard Word versions.

Care Home Nutrition Support Referrals

We are now accepting referrals direct from carers of residents in care homes for nutrition support referrals. We ask carers to follow the guidance in our MUST care pathway for Care Homes and if further dietetic support is needed we ask they complete the Community Dietetic Referral Form for Care Homes. The form can be downloaded below or from our Information for Care Homes section.

Paediatric Community Dietetic Services

GP’s and health care professionals can refer directly to the Paediatric community dietetic service using the downloadable referral form below.

Family and carers can also be signposted to our self-management website www.patientwebinars.co.uk where appropriate. We currently have self management resources (webinars, videos and downloadable resources) for the following paediatric conditions:

  • Fussy eating in children
  • Food allergy in children
  • Weight management in children

We also enable patients to self-refer using the form on our website for the following conditions:
